When applying for the Smart ID Card the applicant needs to bring an original and a photocopy of their green bar-coded ID book.
If applicable (not a 16-year-old first time applicant or 60 + years) a fee of R140 is charged.
The applicant’s latest contact details are also needed so that the office can inform the applicant when the card is ready for collection.
If the Smart Card is to replace a lost or stolen identity document, then an affidavit is required.
Spousal details including names, surname, date of birth, date of marriage and identity number are required.
If the spouse is a non-South African citizen, their permanent residence permit/passport number and the expiry date is required.
At present, non-South African citizen permit holders cannot apply for a Smart Card ID.
