It is alleged that a Toyota Corolla was travelling from Nwamankena to Giyani while the police van in the direction in the direction of Nwamankena (Tzaneen) from Giyani. A taxi travelling in front of the Corolla suddenly stopped on the road to pick up a passenger. The driver of the Corolla swerved to another lane to avoid colliding with the taxi.

The Corolla collided head-on with the oncoming police vehicle. The driver of the Toyota and the two police officers were transported to a nearby hospital for medical treatment.
