2. Think of any possible “gift” or item you think will help CMR Middelburg make a difference.
3. Write in an item under each day.
4. Fill in your personal or company details at the bottom of the page.
5. Return completed form to admin@cmrmid.co.za or Fax to E-mail: 086 415 6582
6. Challenge family, friends or companies to also be part of the challenge.
7. CMR Middelburg will contact you for arrangements regarding collection.
