Hospital is looking or missing patient
Anyone who might have any information or has seen him, can contact the hospital’s Acting Nursing Manager, Mr Jack Radebe, on 084-437-1793 /051-405-9311.
Universitas Academic Hospital (UAH) requests the assistance of the public in locating a patient, the 45 year old Mr Michael Mofokeng, who went missing from UAH Annex at the National District Hospital on 10 July.
Mr Mofokeng was transferred from Bethlehem to Pelonomi Tertiary Hospital’s Trauma Unit in Bloemfontein, where he was admitted and then discharged several days later.
After being discharged from Pelonomi, he was then sent to the eye clinic at Annex via the ambulance.
However, he apparently disappeared after a patient record file was opened for him.
