Hospital needs help to locate patient’s family
Israel Modise Chauke born June 5, 1967, was brought to Ventersdorp community health centre on June 24, 2021, by Emergency Medical Rescue personnel with a history of having collapsed after coming from his workplace at the crèche as a general worker.
Israel Modise Chauke born June 5, 1967, was brought to Ventersdorp community health centre on June 24, 2021, by Emergency Medical Rescue personnel with a history of having collapsed after coming from his workplace at the crèche as a general worker.
According to the community health centre, Israel was transferred to Witrand Rehabilitation Unit via Potchefstroom hospital, where he was unable to provide a clear history regarding his family’s whereabouts as he has speech difficulties and has a physical impairment.
Israel was then transferred back to Ventersdorp community centre on August 26, 2021. According to history provided by members from the community in Goedgevonden, the patient has been staying alone at House No: 206 Goedgevonden village, Venterdsorp, 2710. They add that he was apparently staying at the informal settlement in Ikageng, Potchefstroom where it is alleged that his family might also be staying around the town.
Attempts to trace the family were made at Home Affairs using the patient’s identity document No: 670605 5976 084 but was unsuccessful, as the personnel reported that their system indicates that the ID is blocked and possibilities are:
There might be someone using the same ID number, due to identity theft, or the patient might be from a foreign country. His colleagues at the crèche also have no information regarding the whereabouts of his family.
