The accident occurred when a white Volkswagen Golf Chico and a three tonner truck were involved in a head-on collision on the R555 (Old Witbank Road) between the weighbridge and the train bridge.

It is believed that the Volkswagen was driving in the direction of Middelburg and the truck, carrying residential possessions believed to be part of a house move, was travelling in the direction of eMalahleni. Two people, the driver of the Volkswagen and one person from the truck, were certified dead on the scene. Three other people were taken to hospital with serious injuries.
