The collision involved a truck and a light motor vehicle and occurred roughly ten kilometres out of Middelburg. The car was struck so violently that the engine was ripped out of the body and landed in one of the lanes. All of the occupants in the car were also killed instantly, including a young child.

The truck left the road and came to a standstill in the field, several hundred metres from the scene. The truck driver survived the accident but was rushed to hospital by ER 24.
